Event Reservation-Request

In order to secure an event on the calendar, this form must be completed.

The following steps should be followed to ensure
proper coordination and maximum success of the proposed event.

  1. Please complete & submit this form at least 60 days prior to event to ensure all necessary modes or coordination can been achieved.
  2. Please do not advertise the event until you have received confirmation.
  3. If there's a speaker please submit with this form. 
  4. Request are reviewed & approved by the Leadership team.
Please note that submitting this form doesn’t mean your request is approved. You'll receive an approval notice from either Leadership Team or our Events Team.
